We are delighted to be able to host an initial cardiac screening session in partnership with The Dylan Quirke Foundation.
Please read the details below:
IMPORTANT NOTE:
1. If you have a pre-existing cardiac condition and/or are under medical care for this then this screening is not suitable.
2. If you are covered by Laya Health Insurance you can avail of cardiac screening through Laya every two years - please use Laya options for screening rather than using one of these screening places.
The screening is open to 12-18 year olds - there is a €20 fee for the screening which normally costs approx €100. We are able to offer a significantly reduced cost as the club is supporting this initiative through your recycling of bottles in our orange ReTurn bins - every plastic bottle/aluminium can returned will contribute to wellness initiatives throughout the club. Turn your bottles & cans with the ReTurn logo into a potentially life saving donation by dropping them into the bins at the club!
To book a screening place:
1. Click on the clubzap link, enter the childs name and your contact details and pay.
2. following payment you will receive a booking link for the medical screening provider (AMS) via email - this is your link so please do not share it as it will cause issues with the booking system and could result in your session being voided.
3. Click on the booking link, register with AMS and complete the booking - places are first come first served so please complete this step as soon as possible but before 28th July if at all possible.
4. Once you have confirmed your place with AMS they will provide details of the appointment time and other details for the screening day.
If there are no spaces available for the 11th August (or you are unable to attend on that date but wish to register interest in having your child(ren) screened) please register interest in the waiting list as soon as possible and we will get in touch with details once another screening clinic can be arranged.
The Dylan Quirke Foundation relies completely on fundraising and we are so grateful to be accepted for one of their screening dates. You can provide a donation on the day of the clinic as they will have a 'Tap to Donate' facility at the clinic (no cash can be accepted).
